Home Health

At Allied Home Health and Hospice, we provide compassionate, skilled care in the comfort of our patients’ homes. Our interdisciplinary team works closely with physicians, hospitals, facilities, and caregivers to develop individualized care plans that promote healing, restore independence, and improve quality of life.


Our services are designed to support patients recovering from illness, surgery, or hospitalization, as well as those managing chronic health conditions. Each patient receives care tailored to their unique needs from licensed professionals dedicated to providing exceptional care.

SKILLED NURSING

Our Skilled Nursing team provides professional care and clinical oversight in the home. Registered nurses and licensed practical nurses work closely with the patient’s provider to monitor health conditions, manage treatments, and educate patients and families to support recovery after illness, surgery, or hospitalization and improve long-term health management. Our goal is to help patients safely heal at home while preventing complications and unnecessary hospital visits.  

 

Nursing Services may include:

• Medication management education
• Disease management and patient education
• Wound care and dressing changes
• Post-surgical care
• Cardiac and respiratory monitoring
• Diabetes management and insulin education
• IV therapy and infusion monitoring
• Catheter care and urinary management
• Ostomy care and education
• Pain management support
• Vital sign monitoring and health assessments
• Coordination of care with physicians and other providers  

PHYSICAL THERAPY

Our Physical Therapy team focuses on improving strength, mobility, balance, and overall physical function. Our licensed physical therapists develop individualized treatment plans to help patients recover from injury, surgery, stroke, or illness. Through therapeutic exercises, gait training, and fall-prevention strategies, we help patients regain independence and move safely within their homes and communities. 

 

Physical Therapy Services may include:

• Strength and endurance training
• Balance and fall prevention programs
• Gait training and walking assistance
• Transfer and mobility training
• Post-surgical rehabilitation
• Joint mobility and flexibility exercises
• Pain management techniques
• Assistive device training (walker, cane, etc.)
• Home safety and mobility assessments
• Education for caregivers on safe mobility techniques

OCCUPATIONAL THERAPY

Our Occupational Therapy team helps patients regain the ability to perform everyday activities such as bathing, dressing, cooking, and other essential tasks. Our occupational therapists evaluate the patient’s home environment and physical abilities to develop personalized strategies that promote independence, safety, and confidence in daily living.


Occupational Therapy Services may include:

• Training for activities of daily living (ADLs) such as bathing and dressing
• Upper body strength and coordination exercises
• Fine motor skill rehabilitation
• Home safety evaluations and recommendations
• Adaptive equipment training
• Energy conservation techniques
• Cognitive and problem-solving strategies
• Fall prevention education
• Training for safe use of adaptive devices

SPEECH THERAPY

Our Speech-Language Pathologists evaluate and treat patients experiencing difficulties with communication, cognition, or swallowing. Therapy focuses on improving the patient’s ability to communicate effectively, think clearly, and safely eat and drink.


Speech-Language Pathologist Services may include:

• Speech and language rehabilitation
• Swallowing evaluations and therapy
• Cognitive therapy for memory and problem-solving
• Communication strategies after stroke or neurological conditions
• Voice therapy
• Safe swallowing techniques and diet recommendations
• Education for caregivers regarding communication and swallowing safety

MEDICAL SOCIAL WORKER

Our Medical Social Workers provide emotional support and help patients and families navigate the social, emotional, and practical challenges associated with illness or recovery. They serve as an important resource for connecting patients with community support and services.


Medical Social Worker Services may include:

• Emotional support and counseling
• Assistance accessing community resources
• Coordination of community services and support programs
• Guidance with advance directives and care planning
• Assistance with financial or insurance concerns
• Caregiver education and support
• Help with transitions of care and discharge planning

HOME HEALTH AIDE

Our Home Health Aides provide personal care and supportive services that help patients remain comfortable and maintain dignity while receiving care at home. Aides work under the supervision of a nurse or therapist and follow the individualized care plan developed for each patient.


Home Health Aide Services may include:

• Assistance with bathing and personal hygiene
• Dressing and grooming assistance
• Mobility and transfer assistance
• Assistance with toileting needs
• Light assistance with daily activities
• Monitoring and reporting changes in condition to the care team
• Support with maintaining a safe and comfortable home environment
